I would say that this is more purpose-driven than design-driven. If you look, the holes are created for bolts. However, this could be the work of a creative repairman. As many of you point out, though, the ears are not correct. Therefore, probably just a workman trying to leave his mark, though not a true Hidden Mickey.

The ears are simply too small to be a Hidden Mickey. Maybe a hidden Duffy? A hidden bear on the beer cart?

That's a tough one because the dimensions aren't right, but it looks deliberate so I voted yes.
